There has been some information in the media recently about data sharing in the NHS. If you would like to opt out of your data being shared for research and planning purposes, please click on the link below for the opt out form and return to the Practice before 23rd June 2021.

Alternatively, you can do this on the NHS App. NHS App -> ‘your health’ tab at the bottom -> ‘choose if data from your health records is shared for research and planning’.
