New Patient Registration
To register with our practice, patients who live within our practice boundary area (W4, TW8, TW9 and SW14) are asked to do the following:
To supply your NHS number. You can can request this from your previous practice.
Bring in some form of identification, with proof of residency at that address such as a driving licence or a document with your name and address on it or a current medical card
Complete a GMS1 registration form. This can be downloaded from the above "Registering Online" tab.
You will be given a Registration Questionnaire on registering at the practice and will be asked to then make an appointment for a 'new patient health check' once your registration is actioned, this generally takes up to 48 hours.
